Fourth well child visit

The fourth well-child visit at 14 weeks is an essential milestone in your baby’s growth and development.     The completed MCP card (Green/Red zone) will be reviewed. If it is not completed, the doctor will assist the mother in doing so and then interpret the results.
  5. Assess Parent-Child Attachment
    Observations will be made regarding the quality of attachment between the baby and parents, along with the parents’ responsiveness to the baby’s cues and movements.
  6. Response to Parent Leaving and Returning
    The baby’s reaction when a parent leaves and returns will be assessed to evaluate emotional development and attachment.
  7. Observe Play and Interaction
    Parents’ spontaneous play and interaction with the baby will be observed. If needed, the doctor may suggest a play activity from the CCD card to encourage engagement.
  8. Review Previous Visit
    The doctor will review notes and instructions from the last visit to ensure any recommendations have been followed.
  9. Checklist Review
    The office staff’s completed checklist will be reviewed to determine the key points to focus on during the consultation.
  10. Health Concerns
    Parents will be asked if they have any specific health concerns regarding their baby.
  11. Urine and Stool Patterns
    The baby’s urine and stool patterns will be reviewed to ensure proper digestive health and function.
  12. Breastfeeding Concerns
    If parents have concerns about breastfeeding, these will be addressed. The doctor will ask about breastfeeding frequency and any potential problems with the breasts or nipples.
  13. Vitamin D Prophylaxis
    It will be checked whether the baby is receiving sufficient vitamin D supplementation.
  14. Parent Confidence
    The doctor will inquire about both parents’ confidence in handling and caring for the baby.
  15. Play and Interaction
    Parents will be asked about how they play with and talk to the baby, emphasizing the importance of bonding.
  16. Baby’s Smile
    The doctor will ask how parents encourage their baby to smile, noting emotional responsiveness.
  17. IAP-PSQ-4 Assessment
    If not done earlier, the IAP-PSQ-4 will be completed during the consultation to assess parenting stress.
  18. Interpret the Growth Chart
    The updated growth chart will be interpreted to monitor the baby’s physical development.
  19. Physical Examination
    A general physical exam will be conducted to check for any abnormalities or health issues.
  20. Systemic Examination
    The doctor will perform a systemic assessment to check the overall health of the baby’s organs.
  21. Neurodevelopmental Assessment
    If any concerns are identified on the MCP card, a neurodevelopmental assessment will be conducted.
  22. Vision Check
    The baby’s vision will be evaluated by checking for red reflex, ocular abnormalities, and whether the baby follows objects up to 180 degrees.
  23. Hearing Assessment
    If not done earlier, a hearing test may be ordered to assess auditory function.
  24. Breastfeeding Guidance
    If breastfeeding is painful or problematic, the doctor will assess attachment, suckling, and positioning to correct any issues.
  25. Check for Neglect
    The doctor will look for any red flags of neglect, such as maternal postpartum depression or indicators of baby battering.
  26. Immunizations
    Your baby will receive vaccinations as per the recommended immunization schedule.
  27. Exclusive Breastfeeding (EBF) Counseling
    The doctor will counsel parents on the importance of exclusive breastfeeding (EBF), highlighting its benefits and the potential consequences of not practicing it. Guidance will also be provided on maternal diet and necessary supplements.
  28. Developmental Stimulation
    Developmental activities appropriate for the baby’s age, as outlined in the MCP card, will be encouraged.
  29. Anticipatory Guidance
    Parents will be advised on activities that support the baby’s development:
  • Visual and Motor Stimulation: Provide opportunities for your baby to see, hear, feel, move their arms and legs freely, and touch you. Move bright-colored objects or black-and-white patterns for the baby to see and reach for. Hang colorful, shiny objects 8-10 inches from the baby’s face.
  • Auditory Stimulation: Talk to and smile at the baby. Play soft music, sing or hum, and copy the baby’s sounds. Introduce gentle sounds from different directions.
  • Tactile Stimulation: Gently stroke, massage, and passively move the baby’s limbs. Ensure supervised tummy time to strengthen muscles and coordination.
  • Textures: Let the baby hold and touch different textures, such as paper or various types of fabrics, and mimic the baby’s facial expressions.
  1. Responding to Baby’s Cues
    Parents will be guided on recognizing and responding appropriately to the baby’s cues, including feeding when hungry, soothing when overstimulated, and rocking the baby to sleep.
  2. Preventive Health Care
    Parents will receive advice on maintaining personal hygiene and skin care for the baby, along with a discussion of danger signs that require medical attention.
  3. Next Immunization
    The doctor will provide information about the next scheduled immunization.
  4. Safety Tips
    Parents will be advised not to carry the baby while holding hot food or drinks to prevent burns. Additionally, they should remain vigilant about choking hazards, especially if there are older siblings in the household.
  5. Breastfeeding Check
    The doctor will ask about any difficulties with breastfeeding, the frequency of feeds, and any alternative feeding methods being used. Breast or nipple pain during feeding will also be addressed.
